In this case, Konno aortic root enlargement was found to be the perfect solution to deal with an intraventricular septal aneurysm.

The patient is a thirty-nine-year-old woman with a history of aortic valve endocarditis, which was treated with a ventricular septum patch for a small ventricular septal defect, and a 21 mm bioprosthetic aortic valve replacement two years prior. She was then found to have an aneurysm in the ventricular septum and bioprosthetic valve deterioration, severe tricuspid regurgitation, and worsening preserved EF heart failure.
