Two-Port GelPOINT Carbon Dioxide-Aided Right VATS Lower Bilobectomy for Advanced Non-Small Cell Lung Cancer
Here, the authors describe the safe and effective use of a GelPOINT Alexis® wound retractor (Applied Medical) during a thoracoscopic CO2-aided advanced pulmonary resection. The procedure carried out was a right lower bilobectomy for non-small cell lung cancer (NSCLC). The authors applied a two-port anterior approach, using a 3 cm incision for the utility port at the level of the fourth intercostal space in the anterior axillary line and a further 1 cm incision at the level of the seventh intercostal space in the anterior axillary line. The GelPOINT wound retractor was inserted at the utility port, and it allowed the use of enhanced view and increased surgical space from CO2 insufflation together with the advantages of a 3 cm incision. The postoperative course was uneventful and the patient was discharged home on day two after surgery. Histology confirmed a fully-resected T2aN1 R0 NSCLC adenocarcinoma.
