Robotic Right Lower Lobectomy With Subxiphoid Utility Port

The authors demonstrate a robotic right lower lobectomy with utilization of subxiphoid assistant port. The patient was a 73-year-old man who underwent surgery for a 35 mm lung nodule secondary to metastatic adenocarcinoma of the colon.
A station 7 lymphadenectomy was performed along with taking multiple station 10 samples. The right lower lobe pulmonary artery was divided using a tan 45 (gold tip with a leader catheter) reload and Covidien Signia™ powered stapler. The right lower lobe pulmonary vein was divided using a standard tan 45 reload and the posterior oblique fissure was dissected using a purple 60 reload. The division of the bronchus to the right lower lobe was completed using a purple 45 reload. Samples of the specimen were obtained, and a large quantity of blood was drained from the specimen to allow for a smaller subxiphoid port incision. The procedure was completed with the insertion of an extrapleural catheter and a single chest drain via the subxiphoid assistant port.
