The patient was a 35-year-old woman. She had a cardiovascular history of obesity and an ischemic stroke with a right brachiocrural focus and expressive aphasia in 2012-2013. She continues to have a slight sequel in speech. The transthoracic echocardiogram showed a pedicled image adhered to the ventricular surface of segment P2 of the mitral valve posterior leaflet. The authors decided to perform a resection of this fibroelastoma by a minimally invasive approach. They made a small anterolateral incision in the fourth right intercostal space, used HTK monodose cardioplegia, and carried out arterial and venous cannulation via the femoral route. The patient had a good recovery without complications. No transfusions or inotropic drugs were required. She stayed in intensive care for 24 hours and was discharged on the fourth day after surgery.
